1. IDENTIFICATION
Adult - Bring one of the following for each related adult in the household
Driver’s license, I.D. card, passport/visa, immigration documentsChild - Bring one of the following for each child in the household
Birth certificate, school record
2. PROOF OF INCOME
Bring at least one of the following:

Last month’s paycheck stubs, welfare benefit documents (TANF, food stamps), Social
Security documents, tax returns, Medicaid form
3. PROOF OF ADDRESS
- Mail with your name, lease or house payment, voter registration, utility bill
